Polk County Health and Human Services Agency partners with Blue Ridge Community Health (Blue Ridge) to connect community members with health services. Blue Ridge co-locates in the Public Health offices (161 Walker St., Columbus, NC) to provide a robust list of services (see below). In addition, they operate Collins Dental Services located at 48 Hope Valley Lane, Columbus, NC 28722.
