Community Health Assessments (CHA)
Community health assessment (CHA) is an important part of improving and promoting the health of county residents. A CHA results in a public report, which describes the health indicators, status of the community, recent changes, and necessary changes to reach a community’s desired health-related results.  These reports are prepared every three years in collaboration with local stakeholders and regional partners.
